A three-minute guide to using the checklist

Minute 1: gather, do not diagnose

Bring existing results with dates, units and any instructions you received. Mark missing information as unknown. Choose the one concern you most want to discuss instead of trying to interpret every number yourself.

Minute 2: ask for context

Try: “What does this result mean for me?” and “Would another test change a decision about my care?” Bring an accurate medicine and supplement list to your appointment. Ask what can affect the measurement.

Minute 3: leave with a next step

Confirm what to do, when to follow up and which symptoms or results need earlier contact. Ask whom to contact if questions arise. Keep the plan with your records; do not upload private health information to the website.

Follow existing care instructions. Do not postpone urgent care to complete a worksheet, or change medicines based on the kit.

Locate your existing health records
