No email required. Choose one way to use it:
- Online: Open the browser worksheet, fill in the labelled fields, then print or download your entries before leaving.
- Type in the PDF: Download the file above, open it in a PDF reader, fill in the fields and save your completed copy before closing.
- Write by hand: Print only the pages you need and fill them in with a pen.
Screen readers may not read the PDF in a useful order. The browser worksheet has labelled fields and is the better option for assistive technology.
What you will finish with
One clear action and backup, a record of what happened, and a decision about what to keep, change or pause. You do not need health numbers to begin.

PAGE 3
Notice and decide
Add weekdays and dates, then record an action, backup, skipped or unplanned day. Tick the next-step choices that fit.
PAGE 4
Optional: prepare a conversation
Bring one clear question to an appointment and record the agreed next step.
Start with an example
See a completed example: making time for a friend
What matters: I miss catching up with my friend.
Action: Call Pat for ten minutes on Tuesday and Saturday, after lunch from my living room.
Backup: Send Pat a short voice message.
Obstacle and adjustment: We may not both be free. Agree a time first.
Start and review: 5 October to 18 October 2026.
What helped or was difficult: Tuesday worked; Saturdays were busy.
Did the backup fit? Yes. A voice message helped us stay in touch.
What happened: Two calls worked. Once I used the backup; once I skipped.
Decision: Keep Tuesday; change Saturday to Sunday after breakfast.
Example record starting Monday: – D – – – B – | – D – – – S –. D means did it; B used the backup; S planned but did not do it; – not planned. This is a fictional example.
Choose an ordinary-life priority: a hobby, connection, a calmer evening or another action that suits you. Smaller and less frequent actions count.
Only need appointment preparation?
The optional final page helps you name your main question, decide what to bring and write down the agreed next step. Use records you already have; this is not a testing checklist.
