Three steps to a more useful appointment
1. Gather your numbers
Locate existing reports. Keep dates, units and reference ranges together. Leave unavailable items blank.
2. Highlight anomalies
Mark results already flagged by your laboratory or clinician, plus changes you want to ask about. A flag is a conversation prompt, not a diagnosis.
3. Ask collaborative questions
Choose your priority question. Ask what would change care, what can wait and when to follow up.

Does this replace my doctor’s instructions?
No. The Starter Kit organizes records and questions for a conversation with your clinician. Continue following your care team’s instructions. Do not change medicines, delay care or order tests because of this worksheet.
What if my doctor won’t order specialized longevity tests?
Ask whether the test would change your care, what its limitations are and whether an established alternative answers the same question. Specialized tests are not automatically useful or necessary. You can still use the kit to discuss existing results, everyday habits and an appropriate follow-up plan.
